Indigo 4Ms in primary health care
This project is implementing the Indigo 4Ms tool for primary health care workers in five health services across north east Victoria:
- Alpine Health (Myrtleford)
- Beechworth Health Service
- Corryong Health
- Gateway Health
- Tallangatta Health Service
How does it work?
Each health service will implement the Indigo 4Ms tool for primary health care workers in their routine care of older people living in the community across north east Victoria.
The Indigo 4Ms in primary health care project is a collaboration between these health services, led by Beechworth Health Service. The John Richards Centre for Rural Ageing Research is evaluating the project.
The Indigo 4Ms in primary health care project is funded through an Innovative Models of Care grant from the Australian Government.
